CORE (Community Oriented Recovery & Empowerment) is a program designed to provide hands-on support to individuals working towards their goals. Our team helps members secure employment, pursue education, and build meaningful social connections from anywhere that’s comfortable and convenient for them—whether it’s at home, in a park, or another preferred location. We also offer professional in-home services such as medication management and psychotherapy.
What We Offer Our Members:
- Meet with your doctor at home or in a local park to discuss your medication needs.
- Get help finding and keeping a job that suits you, whether it’s in a local store, an office, or even remote work.
- Receive support to earn your GED or driver’s license, opening up new opportunities for education and independence.
- Enjoy a coffee outing with a supportive companion at a new spot in town.
- Take a walk in the park with a peer supporter to enjoy fresh air while discussing your goals.
- Join a local hobby class or workshop with encouragement from our team.
CORE Services Overview:
CORE is divided into four key services, each tailored to address different needs:
CPST (Community Psychiatric Support and Treatment)
Meet with social workers, nurse practitioners, and other professionals in your home or a location of your choice to receive personalized mental health care and support.
PSR (Psychosocial Rehabilitation)
Learn valuable life skills such as time management, organizational habits, and social communication. PSR also focuses on helping members pursue employment and educational goals.
FST (Family Support and Training)
We work with family members and loved ones to help them understand the member’s needs and build a supportive, healthy home environment.
Peer Support
Connect with someone who has walked a similar path and understands your journey. Peer support offers encouragement, guidance, and a sense of camaraderie.
Eligibility: To qualify for CORE services, individuals must:
- Be 21 years or older
- Have active Medicaid coverage
- Be HARP-eligible (a specialized Medicaid plan designed for individuals with behavioral health needs, ask us about it!)
